PLATELET-RICH FIBRIN (PRF)
THE ULTIMATE BLOOD CONCENTRATE by EZPRF

100% natural, PRF is PRP with superhero powers. Produced by a slow spin speed, no separating plug and chemical additives, PRF is now becoming popularized as a replacement and improvement for PRP in the quest for the ultimate autologous blood concentrate in aesthetics. Clinically, there is more volume effect, better skin rejuvenation and longevity of effect with PRF. Patients are more satisfied with their results, which are more obvious to their eye and fewer treatment sessions are required thus increasing the appeal of such treatments to aesthetic seekers.

Not only does PRF contain more platelets per mcL, it also has leukocytes (WBC) & mesenchymal stem cell! The absence of anticoagulant in preparation also allows an all-important fibrin clot to form after injection which allows for a slow release of the growth factors over longer period vs. short-lived effect of PRP. This fibrin matrix of the PRF serves as a three-dimensional scaffold to maintain the platelets at the site of injection (filler like). To better simplify things, PRF yields stronger results because the “liquid gold” is held in place longer to release more stem cells and growth factors.

PRF is applied to the skin by itself, with needling device or following a laser resurfacing treatment. It can also be injected directly into the problematic areas, just like natural dermal fillers. The injected PRF gives additional volume to the skin for a few weeks then continues to rejuvenate the skin after it subsides. This is a great alternative for someone who is looking to try filler but wants a more natural approach. Because the injected product is 100% your own, factors such as smoking, stress and illness can effect results. A series of three treatments can yield results for up to two years.

Platelet therapy is quickly becoming an integral aspect of cosmetic treatments to stimulate collagen production and enhance the efficacy of procedures such as fat transfer, hair loss, lasers and microneedling. Harvested PRF contains very high concentrations of white blood cells, fibrin and a small amount of stem cells found circulating in our bloodstreams. PRF becomes a gel after 15 to 20 minutes of being isolated and immediately upon being injected into the face or body. This allows it to be used for cosmetic volume restoration in addition to topical indications. Typical uses for PRF at Avissa Skin include injection under the eyes with, in or above the mucosa of the lip area for volume or vertical lines, microneedle treatments, and hair preservation and restoration.

PRF FOR UNDER EYE
The under eye is an area of particular interest to us since the majority of patients seeking aesthetic clinics identify it as a priority. Outside of surgery, most aesthetic providers use hyaluronic acid fillers to address the area, which are less than ideal with a high rate of patient and provider dissatisfaction. Hyaluronic acid by nature attracts water; a property ill desired in the periocular region. Platelet-Rich Fibrin injected to form a fibrin clot used as a stand-alone or combined with fillers represents an alternative leading to high patient satisfaction.

PRF + MICRONEEDLING
Microneedling, or collagen induction therapy, is a popular, minimal-downtime procedure known to improve skin texture, the presence of fine lines and scars, and enhance skin appearance without risks of hyperpigmentation. This superficial, controlled puncturing of the skin by very fine needles stimulates the wound healing response, ultimately leading to the release of growth factors by platelets and collagenesis. Topical application of growth factors has been shown to improve
skin texture and appearance; thus, applying either PRP or PRF topically during and immediately after microneedling treatment can improve results by saturating the newly created fine, porous wounds with concentrated growth factors.

PRF + LASER RESURFACING
Laser resurfacing serves to retexturize the skin by stimulating the body’s wound healing and tissue regeneration response. Topical application of PRP or PRF following such treatments may serve to both reduce healing time and enhance results by further supplementing the tissue injury response with concentrated platelets and their multifunctional growth factors.

PRF FOR HAIR LOSS
If you are experiencing hair loss, you’re basically a good candidate for PRF hair restoration. PRF hair restoration works best when the patient notices early hair loss as growth factors, white blood cells, stem cells, and platelets in the PRF work best when awakening dormant hair follicles. These hair follicles are still healthy and capable of growing hair, but they are spending more and more time in dormant phase. PRF is also effective for women with traction alopecia, hair thinning caused by hairstyles that pulled the hair to an extreme degree and inhibited hair growth.

WHAT TO EXPECT

PRF is harvested from our very own blood. Prior to the treatment, one or two vials of blood is drawn. Next, the blood is placed inside a specialized centrifuge designed to separate PRF from other blood components. After spinning, the PRF is harvested
from the top of the vial into a syringe.  In my clinic, PRF has been most commonly used in concert with hyaluronic acid dermal fillers to enhance facial volume and improve the quality of the surrounding skin.

Depending on how many areas you want to get treated, a volume of blood will be taken. This is usually between 10 to 30 ml. Your blood is then placed in a centrifuge for around 5-8 minutes. Once the PRF is ready, there is a delicate window of approximately 15 minutes before the PRF forms into a gel-like consistency. The PRF is extracted and injected or topically applied during a microneedling or laser resurfacing treatment on the skin. Because PRF is produced by your own body, these treatments are extremely safe. Be sure to drink lots of water before the appointment so we can get a good volume of blood.

If you are getting a PRF treatment for the face, the procedure is very similar to a dermal filler treatment or any other injectable treatment except all the elements being injected are completely natural and extracted from your own body.

HOW MANY SESSIONS?

PRF is performed in a series of treatments, spaced six to eight weeks apart. Results typically last one to four weeks and the volume will essentially dissipate. Long-term effects, such as skin tightening, collagen production and enhanced skin quality, may take up to three to four months to be noticed and can last from six to 12 months. The visibility of expected benefits and results of the PRF treatment is typically seen after six weeks from initial treatment.

The number of platelet-rich plasma or platelet-rich fibrin treatments are dependent on the area that is being treated as well as the condition that is being addressed, including whether your treatment is being performed as a microneedling or injection. We will discuss this with you at your complimentary consultation.

TREATMENT RISK

Because PRP/PRF treatments essentially use elements that are naturally produced by your own body, these treatments are extremely safe. As no foreign substances are being injected into your body, there is no risk of an allergic reaction.

As with all injections, there may be a risk of bruising, pain, and redness caused by the needle used to inject the product. Following post-treatment instructions will minimize this risk.

CONTRAINDICATIONS

You should not get PRP/PRF treatments if you are pregnant or breastfeeding, if you have low platelets, if you have an active infection in the treatment area, certain skin diseases, a metastatic disease, or any blood or bleeding disorders. Patients on “anti-clotting” medications (like Aspirin and other blood thinners) or who have certain autoimmune disorders are not good candidates for platelet-rich plasma or platelet-rich fibrin treatments.

Because PRP/PRF treatments essentially use elements that are naturally produced by your own body, these treatments are extremely safe. As no foreign substances are being injected into your body, there is no risk of an allergic reaction.

PRE TREATMENT CARE

This treatment is highly dependable on the level of hydration of the patient (patients need to start the hydration process 72 hours before treatment). Patients who smoke may have less response to this treatment. Hormone therapy may lead to faster clotting of PRF. Avoid Alcohol and smoking as much as possible for 2-3 days prior to your scheduled treatment.

PRP/PRF is effective in most cases; no guarantees can be made that a specific patient will benefit from this procedure which is why a consultation is so important. Additionally, the nature of cosmetic procedure may require a patient to return for numerous visits in order to achieve the desired results or to determine whether PRP/PRF may not be completely effective at treating the particular condition. The average patient needs about 3-4 treatment sessions separated by 4-6 weeks and the final result won’t be readily visible for 4-9 months.

For injections, bruising and swelling are possible, plan 2 weeks ahead of any events/vacations.

POST TREATMENT CARE

As with all injections, there may be a risk of bruising, pain, and redness caused by the needle used to inject the product. Following post-treatment instructions will minimize this risk. In general you can expect not to wash your face/hair for ideally a 24 hour period following your treatment.
